How to File Late Registration?
Here’s the step-by-step guide on how you can obtain a file late registration.
Step 1: Request a copy of your Document
Request a copy of the PSA documents (birth certificate, marriage certificate, or death certificate) online or walk in.
If the PSA cannot find the document in its records, a Negative Certification or Certificate of No Record will be issued.
Step 2: Filing of Late Registration
After obtaining the Certificate of No Record, late registration should be filed at the LCRO of the city or municipality where the life event occurred. Submit the certificate together with other necessary documents. After paying all the fees, get the Official Receipt (OR) and wait for the document to be released.
Step 3: Endorsement to PSA
The LCR will endorse the registered copy to the PSA. You may ask the LCRO for the timeline of the endorsement for you to know when you should file a request to the PSA.
Upon the endorsement, ask for the copies from the LCRO; Receipt of Forwarder/Courier in case the endorsement was sent to PSA through a local courier, Personal copy of the document from the LCRO, and Copy of the Endorsement Letter or Transmittal Letter from the LCRO to the PSA
